Welcome to Our Lady of Grace Catholic Church Whole Family
 Faith Formation Program!


 Our family centered programs involve both the student and at least one parent
 attending each session

 All information is kept confidential

 When you complete this online registration you will be forwarded to Our Sunday
 Visitor to make your registration payment. Registration payment can be made
 through Our Sunday Visitor or in person at the Parish Office.

 Registration fee: $120.00 per child and for each additional sibling in the family is $60.00. Early
 bird Discount of $20.00 given if registered by June 30, 2025. Registration is open until
 September 15, 2025.

Class Registration Faith Formation Classes Offered for 2025-2026:
Whole Family Year 1: For students in grades 1st-5th who have not received First Communion
First Communion: For students who have completed Whole Family Year 1 at OLG or at
another parish and are entering 2nd-5th grade
Whole Family Program: For students who have received First Communion
Our family understands that ALL Sacrament Preparation activities are required.
We will follow the program rules and understand the guidelines regarding missed gatherings.
In case of an emergency, I will contact the Evangelization Office to make them aware of an absence.
If you are NOT a registered member of Our Lady of Grace Church, please provide a letter from the Pastor of your current church acknowledging your participation in OLG's Sacrament Preparation Program prior to starting class.
All students must submit a copy of the Baptismal certificate prior to receiving sacraments. If you have not submitted a copy of your child's Baptism certificate, please do so NO LATER than 10/1/2025. Baptismal certificates may be obtained by contacting the parish where the Baptism took place.
